Grim Dawn only saves when you exit to the main menu or touch a Riftgate. If you killed a boss, didn't pick up the quest item, and then died, the tracker might still say "Kill the Boss" but the boss is dead. Solution: Reload the game. The boss will respawn (if it's a non-unique enemy).
